A. Your copay is due at the time services are rendered.

A. Monday thru Friday - 8:30 am to 5:00 pm

A. Monday thru Friday - 9:00 am to 4:00 pm

Q. How do I obtain a copy of my medical records?

A. You must sign a medical release form. If your records are being sent to anyone other than a physician or your current insurance company, there is a charge of $20.00 for the first forty pages and 25¢ for each additional page after forty.

Q. How do I get a current prescription refilled?

A. Call our nurses line between the hours of 9:00 AM - 3:00 PM. After checking with your physician, if your prescription can be refilled it should be called in to your pharmacy within 1 to 2 business days.
