Welcome to Generations ObGyn. Here at Generations, our practice consists of seven physicians, a nurse practitioner, four ultra-sonographers, an aesthetician, and many more highly dedicated and caring employees who hope to create a warm, welcoming environment.

Our staff works hard every day to accommodate a large number of patients with a variety of appointments. Please allow plenty of time for parking, registration and paperwork prior to your appointment. Some paperwork can now be done online prior to your appointment via our patient portal. Please bring an up-to-date insurance card and photo ID. Due to the number of patients we see per day, we do have to adhere to a schedule. If you are more than 20 minutes late for your appointment, you may be asked to reschedule.

PATIENT PRIVACY
Generations ObGyn considers your medical records confidential. We will only release the records or any part of them if you give us written authorization. No matter what the circumstances, we will not release these records unless we have written notification that you wish us to do so.

TEST RESULTS
In order to provide a higher level of service to our patients, we are implementing LabCalls, a personalized test result system.
LabCalls will provide you with your test results in a time most convenient for you to retrieve them. You can call our test result number (1-877-678-1215) or visit www.mytestresults.com to obtain your test results.

Allow about 2 weeks for lab results, and about 3 weeks for pap smears.

When we perform certain medical tests for you, you will receive a Unique Identification Number (UID), which is your Social Security Number, and a Personal Identification Number (PIN), which is your birthdate. Once the test results have returned, you can call our test result number or visit our website, 24 hours a day, 7 days a week to hear your results by using your UID and PIN and our customer number (545499).

Q. How do I obtain a copy of my medical records?
A. You must sign a medical release form. If your records are being sent to anyone other than a physician or your current insurance company, there is a charge of $20.00 for the first 40 pages and 25¢ for each additional page after 40.

Q. How do I get a current prescription refilled?
A. We ask that you contact your pharmacy. They will forward a request for your prescription refill. Please allow 24 hours for the request to be complete.

APPOINTMENT REMINDERS

We have implemented a new appointment reminder system (HouseCalls™) to provide our patients with timely, efficient reminders of their upcoming appointments.

HouseCalls™ will call you at home a few days prior to your next visit to confirm the day, date and time of your appointment. You will be able to confirm your appointment by using your telephone keypad and pressing the “1” key to confirm, the “2” key to replay or the “3” key to leave a message for our office. If you are unavailable when the call is made and have voicemail or an answering machine, the system will leave you a brief message.

INSURANCE
Generations ObGyn accepts insurance through many different PPO and HMO plans and accepts traditional indemnity insurance. If you are not sure about our provider status with your insurance company, please check with your insurance company or with our business office.

We also accept major credit cards and participate in the CareCredit healthcare payment program.

PRESCRIPTION REFILLS
Drug prescription refills are filled only during office hours when the physician and nurse have access to your medical record. We ask that you contact your pharmacy. They will forward a request for your prescription refill. Please allow 24 hours for the request to be complete.
