Patient Information

Welcome to Generations ObGyn. Here at Generations, our practice consists of seven physicians, a nurse practitioner, four ultra-sonographers, an aesthetician, and many more highly dedicated and caring employees who hope to create a warm, welcoming environment.

Our staff works hard every day to accommodate a large number of patients with a variety of appointments. Please allow plenty of time for parking, registration and paperwork prior to your appointment. Some paperwork can now be done online prior to your appointment via our patient portal. Please bring an up-to-date insurance card and photo ID. Due to the number of patients we see per day, we do have to adhere to a schedule. If you are more than 20 minutes late for your appointment, you may be asked to reschedule.

Patient Forms

Download the forms below to keep your patient record updated, learn about what constitutes an obstetrical emergency and what over-the-counter medications are safe to take during your pregnancy.

Patient Privacy

Generations ObGyn considers your medical records confidential. We will only release the records or any part of them if you give us written authorization. No matter what the circumstances, we will not release these records unless we have written notification that you wish us to do so.

Test Results

We normally use LabCorp for processing all ordered labs and any pap smears performed in office. To obtain test results, there are two available options: 1) go to www.labcorp.com and create an account to obtain your test results or 2) utilize your Patient Portal with Generations ObGyn. If you are not registered on the Patient Portal with Generations ObGyn, please contact our office to request access: (865) 769-4444.

Please allow up to 2 weeks for lab results and up to 3 weeks for pap smear results.

Normally, we will not call if all results return with Normal findings. If any Abnormal findings occur, you will be contacted by our office after the ordering provider has reviewed the test results and provided direction on next steps.

Appointment Reminders

You will receive an appointment reminder 2-3 days prior to your scheduled appointment utilizing the method you requested at our office-text, email, or phone call. Upon receiving the Appointment Reminder, you will

have the option to Confirm or Cancel your appointment. If you select the incorrect option, or need to reschedule, please call our office: (865) 769-4444, Option 1. You are able to change your Appointment Reminder
delivery method at any time, just let us know!
